Core Serious and Major Accident Investigation Skills

  • Different types of investigation, their role as the investigator and the need to adopt the correct investigative mindset.
  • Formulate a clear Accident Investigation Plan supported by a logical investigative strategy.
  • ‘Golden Hour’ principle during the initial response.
  • Sources of evidence to help build ‘the big picture’.
  • Ensuring information is assessed objectively and in a structured manner.
  • How to brief senior management during the investigation.
  • Working with other agencies who may also be investigating the incident.
  • The need to create an investigator’s ‘Policy File’ to show the decisions made and why.

Investigative Interviewing
  • Different types of interviews and the core principles of investigative interviewing following an accident.
  • Practicalities for gaining initial accounts in the immediate aftermath.
    The PEACE model, questioning skills and the funnelling technique.
  • Management of formal interviews
  • Dealing with ad hoc real-time situational interviews with both employees and non-employees.
  • Dealing with union representatives and other support mechanisms.
    Simulated practical interviewing role plays to practise techniques and gain confidence.

Causation, Human Factors and Report Writing
  • Understanding the difference between cause and blame.
  • Understanding Individual, Job/Workplace and Organisational/Management factors that could have contributed to the incident.
  • Using root-cause analysis theory to combine evidence and indirect human factors to gain an accurate ‘big picture’.
  • Creating a balanced, objective and defensible Investigation report
Modules 11-12
Courtroom Skills
  • Understanding the various outcomes following a serious or major accident and the legal courts/tribunals where you may have to account for investigation.
  • The role of the organisation’s Health and Safety representative as a witness
    The ‘batting order’ of being questioned as a witness.
  • Cross-examination techniques they will encounter when giving evidence and how to handle them effectively.
  • How to give clear, honest and coherent evidence under cross-examination.
  • Mock legal hearings where participants will present the evidence of their findings following the major accident investigation from the simulated exercise.
